Why MSU Needed ?
In a state like Rajasthan, which has a difficult geographical distribution with large portion of it being a desert, health has always been a problem, besides basic needs, despite of primary health centre concepts. State could not get out of its status of being “BMARU” (The sick states ) due to its poverty and illiteracy.
Against all odds, Govt of Rajasthan did its best to render medical facilities to common people; still, surgical services were confined to certain centres like medical colleges and few district hospitals.
Mobile Surgical unit, with the concept of surgical assistance at door level, is well equipped with materials and methods to provide diagnostic, outdoor, indoor and operative facilities under one cover in shortest of time and free of cost at a large scale.
Pre-operative Festival: -
Surgical outdoor consults more than one thousand a day, admits one third of them, investigating with best of diagnostics modalities free of cost.
This scenario like a festival continues for 2-3 days, sometimes longer.
Preoperative management of such a large gathering cannot be seen in any institute, not even in medical colleges.
Major cases like cholelithiasis, urinary tract stones, appendicitis, hernias, hydrocoele, intra and extra abdominal lumps, anorectal diseases which come across in bulk are diagnosed by best clinical skills with help of investigations in shortest time.
Minor cases like swellings anywhere sorted out to deal after the major cases.
Operative Battlefield: -
Number of surgeons with the
help of anaesthetists and paramedical staff carry out the job in an untiring manner, is a unique example of team work similarising the
attitudes at battlefield.
This atmosphere of surgery beats all leading surgical institutes in working capacity attributed by help of God, and willpower to serve the poorest.
Though Arranged in Temporarily built structures standards of sterilization are maintained very high and patients are monitored constantly.
Post Operative Society:-
By the times, it becomes a society of doctors, staff, patients, organisers, villagers and attendants.
Helping operated Patients and each other seems like a social service.
Outcome of surgeries are excellent by blessings of God as no patients have been referred further immediately for any surgical mishappening
Leaving very few exceptions, most of the patients enjoy being operated.
Managing postoperative patients is a lifetime experience, how they are discharged one by one after satisfactory dressings as days passby, finally leaving the staff alone with the sense of happiness and satisfaction
Infection rates are noted remarkably lowest, which otherwise are expected higher when operated at such a large scale and this attributes perhaps to quality, attitude and team work. I memorise less than 0.1% cases got infected out of more than 2000 dealt postoperatively.
Morbidity and mortality rates are even lower than what is mentioned in the textbooks. None of such cases were reported since I joined MSU in 2004 -05.
Feedback Follow Ups:-
A system of follow up has been set up for operated patients who are reviewed within a month.
Similar feedbacks are also taken from organisers and local doctors so as to improve the services.
Operated patients are our best mode of advertisement which is very well reflected by increasing number of patients in consecutive camps in the area.
People even wait for months, sometime years for such camps and often approach at farthest camps also.
Give and Take Education:- At camps, arranged in various geographical locations, prevalence of region wise diseases are discovered like cholelithiasis in north, hydatid disease in south east and urinary tract stones in west Rajasthan.
Suggestions regarding their dietary habits and change in life style are provided by means of health education for prevention of such diseases.
Suggestions are taken and given to organizers and villages though bilateral communication during camp period.
Dream come true experience:-
As a surgeon born out of a common man it is definitely a dream come true experience to serve the poorest section of society using best working set up free of cost, returning a sum of what we have learnt from them.
We are privileged to watch the real face of society and feel the joy of satisfaction by serving them.
